    Our agency provides a wide range of skilled nursing, therapeutic, home health, homemaker and related services, all delivered by a well qualified, supervised and professional staff of caring individuals.
     
    Receiving care at home is a key step toward achieving positive health outcomes for many people. Home health care services can range from highly skilled and technical nursing care to assistance with the essential activities of daily living. Sophisticated technology is expanding the range of services that can be provided in the home, while offering a safer and more comfortable environment for healing.

     

    Skilled Nursing Services

     

    Registered Nurses provide in-home visits to clients of all ages. Their services may include skilled assessment, planning, monitoring, treatment, symptom management and evaluation in collaboration with your physician and other members of your health care team.

     

    Physical, Occupational and Speech Therapy

     

    Physical, Occupational and Speech therapists offer a variety of skilled services, especially after an illness or surgery, or to promote a safe home environment. All recommendations are developed as part of the client’s overall treatment plan.

     

    Client Education and Teaching

    One of the most important components in every area of home care whether it involves surgery or managing several chronic illnesses.

     

    Before a surgical procedure, we work in partnership with your physician to provide information on preparing for surgery. We offer instruction on post-operative care, proper use of assistive devices, and ensuring a safe home environment.

     

    After surgery, our team is available for nursing care, rehabilitation therapy, support services and personal care.

     

    Chronic disease management consulting is available for clients and their families to help with a better understanding of disease process, treatments, equipment, medications and nutritional needs.

    Medical Social Services

    Medical social workers work as members of the health care team in every area of home care. They offer support, resources and planning assistance. They can help families explore options, consider financial implications, and provide emotional support. Their services are designed to improve the coping skills of the family or other caregivers.

    Behavioral Health Services

    People faced with life-changing health problems, such as cancer, diabetes, heart disease, stroke, or Parkinson’s disease, may become depressed. They may worry about how the illness will change their lives. They might be tired and unable to cope with things that make them sad. Treatment can help people manage their symptoms and improve their quality of life.

     

    If you think you or a loved one might be “stuck in a rut” of sadness for too long, it is important to seek help. You can access care for depression through a physcian referral to our home heath agency, so don't delay.

    Hospice & Palliative Care Services

    Hospice programs make it possible for people to spend the final time of their lives at home with support from physicians, nurses, spiritual caregivers, social workers, home health aides and volunteers. Palliative care is offered by a specially trained interdisciplinary clinical team with skills in symptom management.
