The Intake Coordinator processes and triages referrals for home care services. He/she coordinates admission to the appropriate program with the team leaders. Must maintain expertise in clinical guidelines, rules and regulations that govern the industry and possess a sound knowledge of the clinical issues of the healthcare industry and in the requirements of payers and certifying bodies.
As part of the intake process you will be responsible for verifying patient insurance coverage and obtaining prior authorizations as needed, to ensure necessary procedures are covered by an individual’s provider. You will be responsible for entering data in an accurate manner and to update patient benefit information in the organization’s electronic medical record and verify that existing information is accurate. This position requires professionals to submit insurance verifications via insurance portals and by telephone.
Responsibilities:
- Ability to effectively elicit/provide information to and from providers and referral sources (including, but not limited to, supervisors, co-workers, clients) via strong communication skills.
- Captures patient intake referral information and communicating information to appropriate management teams.
- Enters all appropriate patient information per company policies and procedures into electronic medical record including face to face documentation.
- Processes and documents incoming patient needs and requests for services.
- Correctly informs patients of services available.
- Collaborates with scheduler to schedule appropriate personnel in a timely manner without lapses or delays in service.
- Must have a pleasant telephone voice and overall upbeat positive attitude.
- Able to handle difficult situations with diplomacy and tact.
- Assisting clinicians with collecting supplies and other needs as requested.
Requirements:
- High School Degree (or equivalent).
- Knowledge of Home Care preferred
- Medical terminology preferred
- Must be dependable and punctual.
- Must be a self-starter with the ability to meet deadlines and excel in a fast-paced team environment
- Must maintain patient confidentiality.
- Familiarity with Medicare, Medicaid and other third party payer’s guidelines preferred.
- To perform this position successfully, an individual must have an understanding of office procedures, adhere to time frames and anticipate needs.
- Absolute integrity and sufficient maturity to handle the confidential aspects of the work and the ability to get along with both non-professional and professional personnel
- Computer knowledge essential (Microsoft Office Products knowledge a must.
- Position is a year round Monday through Friday (40 hrs.). Hours are from 7:30 am to 4:00 pm. (includes a ½ hour unpaid lunch.

- While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard reach, stoop kneel to install computer equipment

- Must be able to lift up to 15 – 25 pounds at times.
