Visiting Nurse Home Care & Hospice of Carroll County is hiring full-time Licensed Practical Nurses to provide trusted, compassionate and expert home care in our community. New Grad’s Welcome to Apply!     (Part-time and per diem positions also available)

We are seeking to hire Licensed Practical Nurses (LPNs) The nurse’s responsibility includes collecting client health data and working with the patient/family in the patient’s home. Must work closely with RN’s and teams to achieve goals utilizing standard nursing best practice: implementation of plan of care as established by a Registered Nurse, and within scope of practice defined by the NH Board of Nursing.

Essential Job Functions:

  • Follows appropriate plan of care (POC) developed by the Registered Nurse in partnership with the patient, patient representative (if any) and/or caregiver(s) based on thorough assessment incorporating all aspects of patient limitations and goals.
  • Documents accurate and ongoing treatments: nursing interventions, patient response to care provided, patient/caregiver educational needs, problems, capabilities, limitations and progress toward goals. Documentation includes evidence of appropriate patient/caregiver teaching, and the understanding of these instructions in accordance with agency guidelines
  • Engages the patient /caregiver in meaningful education as indicated per care plan.
  • Communicates with all physicians involved in the POC and other health practitioners related to the current POC.
  • Demonstrates responsibility to effectively communicate the patient’s achievement toward the goals within the POC with members of the interdisciplinary Team and other health professionals through team/patient care conference.
  • Attends Agency In-service programs.

Qualifications:

  • Prior home health experience preferred but we are willing to train new graduates.
  • Tech savvy (preferred)
  • Strong documentation, communication, and symptom management skills (required)
  • Driver’s license in good standing, reliable transportation and adequate vehicle insurance are required
  • Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).
  • Proof of completed COVID-19 vaccine
