Dementia Palliative Care

Our Dementia Palliative Care Program focuses on palliative care for those living with dementia or cognitive impairment in northern Carroll County, New Hampshire and western Maine.

What is palliative care? 

Palliative care is specialized medical care for people with serious illnesses. It focuses on providing patients with relief from the symptoms, pain, and stress of a serious illness—whatever the diagnosis. The goal is to improve quality of life for both the patient and the family. Palliative care is delivered in the patient’s home, care facility, hospital, or at a doctor’s office.

Our Dementia Palliative Care Program is supported by a grant from the Administration for Community Living (ACL).  The ACL’s mission is to:  Maximize the independence, well-being and health of older adults, people with disabilities across the lifespan, and their families and caregivers. The grant to our VNHCH program and to our care partners emphasizes the promotion of a dementia-friendly community.

Our VNHCH’s mission for our program is to improve the quality of life for those in our community living with dementia or cognitive impairment in collaboration with care partners in our community.  Our work is directed to those with these chronic brain conditions and to their family members, caregivers and support people.
