Per Diem Physical Therapist
You will develop and provide care to the patient based on the patient’s needs and goals. Work collaboratively with a team of Home Care professionals to provide quality care and education to the patient. Must have ability to work within a multi-disciplinary team, be well organized and have good communication skills and multi-tasking abilities.
An understanding of the broader health care system ensures patients / clients receive appropriate services in the home environment which best meets their care goals. Home and Hospice Care includes caring for and educating the family unit as they work toward complete recovery or to adjust to an altered lifestyle.
Must have valid NH License (Maine license a plus). Be available for weekend rotation (we are closed on holidays). Background checks will be required. Must have reliable transportation and sufficient auto insurance coverage. Creative thinking is encouraged, iPad experience is desirable, time management is essential and a sense of humor is expected.
